家賃保証会社(やちんほしょうがいしゃ)の審査申込書(しんさもうしこみしょ)ガイド
EN: In Japan, most landlords require you to pass a screening by a "rent guarantor company" (保証会社) before renting. You pay them a fee (usually 50–100% of one month's rent at first, then a small yearly fee), and they guarantee your rent to the landlord. Almost every foreigner renting an apartment must fill out this form. If the screening fails, you cannot rent the room.

Katakana version of your name. If unsure, copy from your bank card or phone contract. 例: グエン バン ア

Company name as written on your contract. 派遣(dispatch) workers: write the 派遣会社 (agency), not the client site
例: 1年6ヶ月. Just started? Write 内定 (job offer) and attach the offer letter
Gross, not take-home. From 源泉徴収票 or estimate: monthly salary × 12 + bonus
Must be a person living in Japan who speaks Japanese if possible (senpai, boss, support organization staff). Ask their permission FIRST — the company may call them
Usually 不要 (not needed) when using a guarantor company — leave blank unless told
The guarantor company WILL call you (and sometimes your employer) within a few days. If you miss the call, screening often fails. Save the agent's number, answer unknown numbers that week, and tell your company's office "電話が来ます" in advance.
If rent is more than about 1/3 of your monthly income, screening often fails. 家賃は月収の3分の1まで。
If your 在留期間 ends within a few months, prepare to show your renewal plan. Short remaining visa is a common rejection reason.
A wrong guess looks like lying. 分からない欄は 空欄にして、不動産屋(ふどうさんや)に聞いてください。
初回保証料 = usually 50–100% of one month's rent. Every year after: 約1万円 (varies). This is NOT returned when you move out.
